This notice is to inform you that as of October 23, 2025, Lisa Brittmon will no longer be practicing at Carevide due to the closure of our Carevide Bonham health center. The decision comes after careful consideration and is the result of multiple factors, including rising operating costs, decreasing organizational funding, and a decline in patient volume.
Carevide is committed to helping patients transition their care. Patients who wish to continue receiving services with Carevide may transfer to Carevide Cooper, which has limited availability for patients. To request a transfer and schedule an appointment, patients may call 903-395-0586.
For those seeking other options, patients are encouraged to contact their insurance provider for local in-network physicians, or use the Health Resources & Services Administration’s Find a Health Center tool (https://findahealthcenter.hrsa.gov) to locate other federally qualified health centers in the area. These centers provide affordable, comprehensive care regardless of insurance status.
Patients who would like their medical records transferred to a new provider may do so through Provider 1st, Carevide’s medical records partner. Requests can be made by calling 855-514-2378, emailing requests@provider1st.com, or faxing 800-818-2114.
